Federico Hernández Alfaro is a Barcelona oral and maxillofacial surgeon and director of the Instituto Maxilofacial. He is the European name GQ used in 2023 when it followed Looksmax.org users into the operating theatre: men paying to have jaws broken for a more “alpha” lower third.[1] The Spanish edition put a reporter in his theatre the same season.[2]

Alfaro’s public pitch is orthofacial rather than “just bite”: reposition jaws to change nose, lips, and neck, not only occlusion. The clinic’s Barcelona Line is a published nasion-perpendicular reference for how far to bring the jaws forward; a British Journal of Oral and Maxillofacial Surgery paper treated it as a planning plane, and the institute markets a more protrusive, angular lower face as the contemporary attractive set.[3] GQ reported on the order of 200 orthofacial cases a year and a doubled international load over five years.[2]
