Cog threads have barbs (cut, molded, or helical) that catch subcutaneous tissue so the operator can hitch a vector — midface, jaw, brow. This is the “lift” in thread lift, as opposed to mono thread collagen mesh. Bidirectional barbs are common; press-molded 360° barbs are the MINT lift pitch. 4D/6D/8D/9D on a pouch usually means how many rows of barbs, not nine operations.
Plane and vector decide whether you get a cheek or a dimple. Migration, visibility, and a thread poking through are the failure modes. Tying paired cogs at one entry is a published trick against migration.
