A custom jawline implant is a one-piece (or designed-as-one) implant from chin through mandibular body to both angles, milled from a 3D CT — silicone, PEKK/PEEK, or similar. It is the looksmaxxing-community's prestige hardmax for the lower third, as opposed to a stock chin implant that only does the front.

Barry Eppley (Carmel / Indianapolis) popularized the "wraparound" language and publishes case studies of male jawline volume. Michael Yaremchuk (Boston) is the other English-forum name for wraparound jaw; long-term threads describe bone overgrowth making later removal ugly. Europe’s published pole is titanium patient-specific implants (Maurice Mommaerts). Brazil has in-house 3D PMMA (João Pedro Mazzon). None of these are endorsed here.

Technique (description)

CT → CAD design (projection, width, vertical length, sometimes a chin cleft) → intraoral + often a small submental incision → subperiosteal pocket → implant. Recovery is weeks of swelling. How much you see depends on fat and skin, not only millimeters on the CAD.

Pitfalls

• Stock chin + stock angles with a gap in the body — the "discontinuous jaw" fail.
• Too much width for the soft-tissue envelope — wound, exposure, unnatural flare.
• Infection; malposition; need for revision.
• Combining with bimaxillary osteotomy or old implants: removal can sag the neck (forum patient logs; treat as anecdote plus a real mechanical problem).
